Your first visit

What to bring, and what to expect

Bring your completed forms, a photo ID and your insurance card, a list of the medications you take, and any referral paperwork or X-rays from your dentist.

At your consultation, Dr. Johnson reviews your health history and imaging and explains your options. If you're scheduled for IV sedation, your care team will give you arrival and preparation instructions in advance — and you'll need a responsible adult to drive you home.

Before-your-visit questions

Do I have to fill out the forms before I arrive?

It helps. Completing the Patient Information Form and Health Questionnaire ahead of time shortens check-in, but you can also fill them out when you arrive.

Is the office accessible?

Yes. The office has a wheelchair-accessible entrance, parking lot and restroom.

Do you offer financing?

Do I need a driver?

If you'll be sedated for your procedure, yes — plan for a responsible adult to drive you home afterward.
